In a study titled "Risk of Preterm Birth Is Reduced with Successful Periodontal Treatment," (from the University of Pennsylvania, Washington University) determined whether a reduction in infant prematurity was associated with successful periodontal treatment. Subjects who were successfully treated for their periodontal disease had a significantly lower incidence of preterm birth less than 35 weeks gestation. MORE

Efficiency And Effectiveness Of Suresmile WJO 2010;11:16–22.

Patients treated using the SureSmile process and a conventional approach were anonymized and randomized prior to independent scoring by two ABO OGS–calibrated examiners. Results: Intra- and interexaminer reliability was consistent in all components with no differences between examiners (r = 0.96, P < .001). The ABO OGS score for the SureSmile patients (mean 26.3) was 4.4 points lower (P < .001) than for those treated conventionally (mean 30.7). Furthermore, treatment with the SureSmile process was shorter (14.7 months vs 20.0 months). Conclusion: The SureSmile process results in a lower mean ABO OGS score and a reduced treatment time than conventional approaches. The approach has great potential to both decrease treatment time and improve quality.

Methods: Three-dimensional images were obtained in a reproducible and controlled environment from a commercially available stereo-photogrammetric camera capture system. The 3dMDface system (3dMD, Atlanta, Ga) photographed 200 subjects from 2 population groups (Budapest, Hungary, and Houston, Tex); each group included 50 men and 50 women, aged 18 to 30 years. Each face was obtained as a facial mesh and orientated along a triangulated axis. All faces
were overlaid, one on top of the other, and a complex mathematical algorithm was used until an average composite face of 1 man and 1 woman was obtained for each subgroup (Hungarian men, Hungarian women, Texas men, and Texas women). These average facial composites were superimposed (men and women) based on a previously validated superimposition method, and the facial differences were quantified.

The clinical concepts regarding the restoration of root canal treated teeth are not clear today and often based on conjecture due to a lack of sound empirical data. The diversity of published opinions is confusing and may lead to less-than-optimal treatment selections. There is also emerging debate whether or not a post is necessary. The purpose of the present article is to organize this topic in evidence-based principles and provide dental practitioners with clear guidelines about restorative therapy for premolar and molar teeth. Two clinical reports are presented, to exemplify the different clinical situations.
The first case highlights a conservative restoration of an endodontically treated molar, maintaining an adequate quantity of healthy residual tissue and only covering the cusp with restoration material, which was not sustained by healthy dentin. In the second case, the fact that the premolar was so badly damaged by carious pathology meant that it was necessary to use a pin to anchor the composite material, and the cusp was covered completely. Scientists uncover genes that influence tooth development
Genes that influence tooth development in the first year of life have been identified by scientists.

The research team has also uncovered a link between the delayed appearance of milk teeth and the need for future corrective work.

The research, which involved teams from Imperial College London, the University of Bristol and the University of Oulu in Finland, has identified several genes which affect tooth development in the first year of life.

They found that babies who carried specific genetic variants tended to have fewer teeth by their first birthday than other children. Full PAPER

apex of the roots in the mandibular arch had more distal inclination than in the maxillary arch.
Conclusions: To minimize root contacts, microimplants need to be inclined distally about 10-20 degrees and placed 0.5 to 2.7mm distally to the contact point to minimize root contact according to sites and levels, except into palatal interradicular bone between the maxillary first and second molars. How to Select Short implants ?
The key to the implants' strength is their width, Dr. Urdaneta said. As long as they are at least 3 mm wide, they do not break, he said. Typically, such implants are embedded about 7.9 mm into the bone, but some are inserted only 5 mm deep. By contrast, long implants can be placed more than 11 mm into bone.
With short implants, "most impressive is what happens below the gum," he said. Some patients who wear these implants for five years have gained more than 1.5 mm of bone, according to Dr. Urdaneta.
Contrary to popular belief, he said, relatively large crown-to-implant ratios don't cause crestal bone loss. He cited a study in the International Journal of Oral and Maxillofacial Implants that compared bone loss of implants with different crown-to-implant ratios (JOMI, January-February 2005, Vol. 20:1, pp. 69-76). "Long implants and/or splinting can result in greater crestal bone loss," the study authors concluded. MORE

PREGNANT WOMEN have been warned not to ignore dental problems following the case of an unnamed woman with the common gum disease gingivitis who delivered a stillborn baby at 39 weeks.
Doctors were able to trace the oral bacteria in the 35-year-old mother’s mouth to bacteria found in the baby’s bloodstream, lungs and stomach.
The research was carried out by microbiologist Yiping Han in the Department of Periodontics at Case Western Reserve University School of Dental Medicine, Ohio.
The findings, published in the February issue of Obstetrics Gynecology , suggest that any disruption to the amniotic fluid could present a risk to both mother and baby – making the infection from gum disease a real risk.

Teeth Show Evidence of Radiation Exposure
Researchers at Howard University in Washington College of Dentistry believe that tooth enamel stores important data about a person’s exposure to radiation. The team is developing Electron Paramagnetic Resonance (EPR) to determine the level of free radicals in substances, including tooth enamel.

What will this information be used for? The hope is that EPR can assist medical workers in triaging patients or dividing victims of radiation exposure into classes by the amount of radiation received. The new technology is minimally-invasive and would provide data useful in treating people exposed to radiation in an accident or by a “dirty bomb,” which refers to a radiation dispersal device. MORE

Late Baby Teeth May Mean More Orthodontic Visits

People with canker sores have much lower intake of vitamin B12 and folate than average, a study concludes.

Researchers at the University of Connecticut collected diet information on 100 people who had a history of canker sores. They compared this group of people to a national survey of more than 9,000 people. These people had completed a diet history, but did not have canker sores.

The canker sore group had much lower intakes of vitamin B12 and folate than the nationwide group without canker sores. Other studies have found vitamin B12 or folate deficiencies in some people with canker sores. In at least one study, people were treated for these deficiencies. Their canker sores either got smaller or disappeared altogether.MORE

Sweet tooth could mean something more serious
Children's response to intense sweet taste is related to both a family history of alcoholism and the child's own self-reports of depression, according to new research from the Monell Chemical Senses Center (Addiction, February 9, 2010).

Because sweet taste and alcohol activate many of the same reward circuits in the brain, the researchers examined the sweet preferences of children with a genetic predisposition to alcoholism. They also studied the influence of depression, hypothesizing that children with depressive symptoms might have a greater affinity for sweets because sweets make them feel better.

In the study, 300 children between 5 and 12 years of age tasted five levels of sucrose (table sugar) in water to determine their most preferred level of sweetness. The children also were asked questions to assess the presence of depressive symptoms, while their mothers reported information on family alcohol use. MORE

Bond Strength of different Adhesive systems

Etch-and-rinse adhesive AdperTM Single Bond 2 yielded high bond strength when applied to both normal and caries-affected dentin. The two-step self-etching adhesive Clearfil SE Bond generated the highest bond strength to ND among all adhesives tested but a significantly reduced strength when applied to CAD. For the one-step self-etching adhesives, Clearfil S3 Bond and iBond GI, the bond strength was relatively low regardless of the dentin type. SEM interfacial analysis revealed that hybrid layers were thicker with poorer resin tag formation and less resin-filled lateral branches in the CAD than in the ND for all the adhesives tested.
Conclusion: The etch-and-rinse adhesive performed more effectively to caries-affected dentin than the self-etching adhesives. Full PAPER

Successful Treatment of Early Implant Failure

The study sample consisted of 18 implants presenting at 4–6 weeks postplacement with early progressive bone loss. Clinical examination indicated the presence of a fistula in the soft tissue covering the implants in most cases. Defects around the implants were curetted, exposed implant surfaces were mechanically debrided and treated with tetracycline solution, and the defects were filled with bone graft and doxycycline powder. Bioabsorbable membranes were used. Final crowns were placed after 6 months. The patients were followed for an average of 30 months.
Conclusions: Early detection and treatment of early progressive bone loss around dental implants are the key to saving early failing implants. The author recommends reevaluation visits 4–6 weeks postimplant placement to detect any signs of early failure so that immediate treatment can be undertaken if needed.

Four groups of premolars (n = 64) were instrumented to size 45/.04 ProFile and filled with laterally compacted gutta-percha cones or Resilon cones in association with AH Plus or Epiphany sealer. The specimens were stored for 2 weeks at 37°C and 100% humidity. After that, leakage was measured using the fluid filtration method and determined as μL/min · 10 psi.
Homogeneity of variance and multiple comparisons tests were performed and indicated that root fillings with the combination of gutta-percha cones and AH Plus sealer showed lower leakage values (P < .05) than the other groups.
Considering the experimental conditions and the results obtained, it was concluded that fillings made with gutta-percha cones in combination with AH Plus sealer had the best performance.

Results showed that a regimen of 30-seconds pre-procedural rinsing with 0.12% chlorhexidine gluconate before dental procedures consistently reduced colony-forming units than without rinsing due to the ability of antiseptic mouthwash to inhibit microbial growth. There was more aerosol contamination during scaling procedures than during the use of a high speed air turbine handpiece. The findings of present study showed statistically significant differences in lip pressure between different orthodontic patients. There was a difference (P = 0.004) in lip pressure between Class I and Class II division 1 subjects. A higher lip pressure exists in males. Lip pressure in subjects with an open lip relationship was lower (P = 0.026) when compared with those with tongue interposition or with no particular habits. The lawsuit says Donna Delgao's surgeon left an inch-long piece of steel in a wound after dental surgery in 2008. The tool lodged in her right maxillary sinus. It was only removed 11 months later by another surgeon.

The objective of this study was to assess the reliability of three-dimensional (3D) cephalometric approaches by comparing this method with authenticated traditional two-dimensional (2D) cephalometry in angular cephalometric measurements.

Conclusions: The 3D angular cephalometric analysis is a fairly reliable method, like the traditional 2D cephalometric analysis. Currently, the 3D system is likely to be more suitable for the diagnosis of cases with complex orthodontic anomalies. Gradual intrusion of the maxillary and mandibular anterior teeth was achieved with a relatively simple orthodontic force system. A significant profile improvement was observed during the 18 months of treatment due to the retraction and intrusion of the incisors in both arches. This intrusion was accomplished without any sign of apical root resorption. The mandibular incisors were uprighted 6.5 degrees, and their maxillary counterparts were uprighted 13.4 degrees. The 2-year follow-up examination revealed a stable result with an increase in periodontal attachment as well as esthetics and function. The effects of lavender scent on dental patient anxiety

In a cluster randomized-controlled trial, patients' (N = 340) anxiety was assessed while waiting for a scheduled dental appointment, either under the odor of lavender or with no odor. Current anxiety, assessed by the brief State Trait Anxiety Indicator (STAI-6), and generalized dental anxiety, assessed by the Modified Dental Anxiety Scale (MDAS) were examined.
Analyses of variance (anovas) showed that although both groups showed similar, moderate levels of generalized dental anxiety (MDAS F(1,338) = 2.17, P > 0.05) the lavender group reported significantly lower current anxiety (STAI: F(1,338) = 74.69, P < 0.001) than the control group.
Conclusions: Although anxiety about future dental visits seems to be unaffected, lavender scent reduces present state of anxiety in dental patients.

Tooth loss and intakes of nutrients and foods: a nationwide survey of Japanese dentists

This study analyzed the data from 20 366 Japanese dentists (mean age ± SD, 52.2 ± 12.1 years; women 8.0%) who participated in a nationwide cohort study from 2001 to 2006. The baseline questionnaire included a validated food-frequency questionnaire to estimate intakes of foods and nutrients. We computed the geometric means of daily intakes by the number of teeth, adjusting for age, sex, smoking, physical activity, and history of diabetes.
Conclusions: Tooth loss was linked with poorer nutrition even among dentists.

In all articles analyzed, the authors described the success rates of miniscrews as suf?cient for orthodontic treatment. This review further showed that: screws of 1.2-mm diameter and at least 8-mm length are preferable, because they are stable and minimize the risk of root damage. The maxilla was shown to be better suited for miniscrews. As for the placement protocol, the data were inconclusive for de?nite recommendations. Arrest in USA after forged prescription

A dental assistant student was arrested on Dec. 21 for allegedly attempting to fill a forged prescription for a narcotic painkiller, police said.
Robert Frega, 24, of Waldwick, was arrested at Walgreen's Pharmacy, 655 Wyckoff Ave., after reportedly attempting to fill a prescription for Percodan pain tablets with a stolen prescription blank.

The purpose of this study was to evaluate changes in dentofacial growth before, during, and after use of a magnetic activator device (MAD) II in high-angle Class II division 1 patients.

The increase in gonial angle, GnGoAr angle, S–N, Co–Gn, Go–Ar, ANS–Me, S–Go, and N–Me distances and the decrease of overjet, U1–NA distance, and ANB angle were statistically significant during the treatment. At T4, there was a decrease in gonial angle and U1–NA distance, which was significant. The changes as a result of treatment were stable at T4.

New Research shows way to Regrow Enamel
When tooth enamel is grown it is bathed in bubble-shaped groupings of proteins, Diekwisch said. The size of the protein bubbles varies in different animals, from 5 nanometers in cows to 20 nanometers in mice and 40 nanometers in frogs.

Diekwisch's team discovered that the longer the stretch of proline repeats, the more the protein bubbles contracted. The study also showed that the smaller protein bubbles were associated with longer enamel crystals, he said. The new discovery, Diekwisch said, will give new clues to engineer tooth enamel.

An endodontic file accidentally swallowed: Clinical and legal implications

The use of a rubber dam is an essential procedure when treating root canals, and not using it may compromise the success of the treatment, as well as the patient's health. This report presents a case in which a K-file was accidentally swallowed during an endodontic procedure, performed without the use of a rubber dam. Given the absence of immediate clinical complications, the subject underwent a chest radiograph. The file was located at the oesophagogastric junction and was later removed by endoscopy. Singapore opens door for foreign Dentists
As part of Singapore's push to be a dental hub, the Ministry of Health will start an accreditation scheme for private dental groups that - for the first time - will allow even dentists without a degree recognised here to treat and manage patients as part of their training.

Australian Dentist fined heavily for fraud on patient
A DENTIST who reconstructed a patient's mouth when the man required work to only three teeth has been ordered to pay more than $345,000 in compensation and interest.

Dr Mark Phung, who treated Mr Dean on numerous occasions in 2002 and 2003, charged his employer's insurance company $73,645 to perform root canals on each of his 28 teeth. In one bill, Dr Phung charged the insurer $28,740 for work performed on two occasions. In another, he charged $10,000 for one session.

Cosmetic dental surgery not reimbursed under CGHS
New Delhi, Dec 15 (PTI) Expenses incurred on treatment of dental diseases are reimbursed to beneficiaries of the Central Government Health Scheme (CGHS) at pre-fixed rates, Minister of State for Health and Family Welfare Dinesh Trivedi said today.

Replying to supplementaries during Question Hour in Rajya Sabha, he said only essential dental treatments are reimbursed but not cosmetic surgeries.

Till now only 10 procedures were allowed for reimbursement but now 55 more procedures are being added, he said.

"Procedures for which there are no CGHS rates, reimbursement is considered as per AIIMS rates," he said adding treatment of asymmetrical teeth of children was not considered for reimbursement as it fell in the category of cosmetic treatment.

Gingivitis, which may affect more than one-half of the U.S. adult population, is a condition commonly attributed to lapses in simple oral hygiene habits. However, a new study shows that development and reversal of gingivitis at the molecular level is apparently much more complicated than its causes might indicate.
Research conducted jointly by the University of North Carolina at Chapel Hill and Procter & Gamble (P&G) Oral Care has found that more than 9,000 genes -- nearly 30 percent of the genes found in the human body are expressed differently during the onset and healing process associated with gingivitis.

Endodontic infections are polymicrobial infections resulting in bone destruction and tooth loss. The host response to these infections is complex, including both innate and adaptive mechanisms. Osteopontin (OPN), a secreted, integrin-binding protein, functions in the regulation of immune responses and enhancement of leucocyte migration.
This study assessed the role of OPN in the host response to endodontic infection using a well-characterized mouse model.
Study concluded that OPN has a protective effect on polymicrobial infection, at least partially because of alterations in phagocyte recruitment and/or persistence at the sites of infection, and that this molecule has a potential therapeutic role in polymicrobial infections.

Despite not having been formally evaluated in the endodontic literature, claims have been made regarding the acceptability of the paper point technique (PPT) in estimating the location of the apical foramen (AF). Our aim was to investigate the repeatability and accuracy of PPT in estimating AF location in a cohort of dental patients.

Root canals with ≤10° of curvature (n = 71) in unsalvageable anterior and premolar teeth were measured using PPT as described in the literature. An endodontic file was cemented in each canal to the position indicated by PPT. Teeth were extracted and microscopic computerized tomography scanned.
The PPT was 0.5 mm short to 0.5 mm long of AF in 87% of the canals. Repeated measurements in a given canal were within 0.19 mm of each other 95% of the time.

For relatively straight root canals, PPT appears to be similar to current clinically acceptable, NOTE- It is a well known fact that most canals are curved even though they appear straight in radiograph.

Paul Patrick Serdula, 47, who worked in dental and medical offices across metro Atlanta, was arrested Monday night on child molestation and sodomy charges. Preventing H1N1 transmission in dental clinic

Tapered Rotary Systems Do not Clean apical third

Mesiobuccal canals of 60 molar teeth with angles of curvature between 25 and 35 degrees were prepared with a torque controlled low speed engine; 30 canals for each system. Each individual instrument was used to prepare four root canals and the time required for preparation was recorded. Standardized radiographs were taken before and following instrumentation and used to determine changes in canal curvature.

SEM analysis of the root canals showed no differences between groups. Clean dentine surfaces were seen in the coronal and middle thirds, but smear layer and debris remained in the apical third of the root canal with both systems. Do periodontopathogens disappear after full-mouth tooth extraction?

The elimination of subgingival niches, by extraction of all natural teeth, resulted in a 3-log reduction of Porphyromonas gingivalis and Tannerella forsythia, and more modest reductions of Aggregatibacter actinomycetemcomitans and Prevotella intermedia. However, the detection frequencies of these periodontopathogens in saliva and on the tongue remained unchanged after full-mouth tooth extraction.

Conclusion: In contrast to what has been believed so far, full-mouth tooth extraction does not result in eradication of all periodontopathogens but only in a significant reduction. The clinical consequences of this observation remain speculative.

MUNGER, Bihar: A non-bailable warrant of arrest was slapped on a private dental surgeon on Tuesday for allegedly cheating an additional district and sessions judge here. Chief judicial magistrate Manoj Kumar Sinha issued the warrant after police said the dentist was evading arrest.

The then Munger additional district and sessions judge, Amarnath Sahay, had lodged an FIR at Kotwali police station in March this year against dental surgeon K K Sinha in this regard.

Researchers have discovered that gum disease can affect the brain in elderly patients.

This can happen by causing inflammation throughout the body, a risk factor for loss of mental function.

The study, based on adults aged 60 and older found those with the highest levels of the gum disease-causing pathogen Porphyromonas gingivalis were three times more likely to have trouble recalling a three-word sequence after a period of time.

The study, led by Dr James Noble at Columbia College of Physicians and Surgeons in New York, also found that adults with the highest levels of this pathogen were two times more likely to fail three-digit reverse subtraction tests. MORE HERE

New Gel to remove tooth decay

A new product made from an Arequipean fruit locally known as "Papaya arequipeña," (Carica pubescens) that allows the removal of tooth decay without the use of mechanical methods, was officially presented by the Interdisciplinary Center for Research and Innovation (CICA) at Arequipa's Catholic University of Santa Maria (UCSM).

This article describes a novel approach to treat smooth-surface white spot lesions micro-invasively. The technique is based on the infiltration of enamel caries lesions with low-viscosity light-curing resins called infiltrants. After the surface layer is eroded, the lesions are desiccated and a resin infiltrant is applied. The resin penetrates into the lesion micro-porosities driven by capillary forces and is subsequently hardened. Infiltrated lesions lose their whitish appearance and look similar to sound enamel. Additionally, the treatment prevents lesion progression. The proposed technique might be an alternative to microabrasion and restorative treatment, in particular for white spot lesions of esthetically relevant teeth.

Dentists Can Identify Patients At Risk For Fatal Cardiovascular Event.

A new study indicates dentists can play a potentially life-saving role in health care by identifying patients at risk of fatal heart attacks and referring them to physicians for further evaluation.

The participating dentists and their staff members used a computerized system, HeartScore (European Society of Cardiology, Sophia Antipolis, France), to calculate the risk, in percentages, of a patient’s dying as a result of a CVD event within the next 10 years. Dentists advised patients with HeartScores of 10 percent or higher to seek medical advice. After six to 12 months, one of the authors conducted structured telephone interviews to ascertain the results of the medical referrals.
Conclusions. Oral health care professionals can identify patients who are unaware of their risk of developing serious complications as a result of CVD and who are in need of medical intervention. The authors’ findings suggest the need for more studies with larger sample populations. More HERE

Low-level laser therapy for pain caused by placement of the first orthodontic archwire

The patients in the low–level laser therapy (LLLT) group had lower mean scores for oral pain and intensity of pain on the most painful day. Also, their pain ended sooner. LLLT did not affect the start of pain perception or alter the most painful day. There was no significant difference in pain symptomatology in the maxillary or mandibular arches in an evaluated parameter.
Conclusions
Based on these findings, it was concluded that LLLT efficiently controls pain caused by the first archwire.

New Filling Material is Born
Kent Coulter and his colleagues at Southwest Research Institute in San Antonio have developed a new proof-of-concept dental restorative material under a program funded by the National Institutes of Health that seeks to replace amalgam with other materials. They will describe the material on November 9 at a meeting of the scientific society AVS in San Jose. The new fillings are made with a plastic-like material containing zirconia nanoplatelets -- tiny crystals made of the same sort of material used to make fake diamonds and gem stones. Unlike their costume jewelry cousins, the zirconia nanoplatelets super hard because of a difference in the particular arrangements of the atoms in the material.

Coulter and his colleagues designed a way to make a roll of this material under vacuum. They envision that this material would be lifted from the roll and packed in a dental cavity and then cured -- using an ultraviolet lamp or some other means -- so that it hardens in place without shrinking. Lung Cancer Signs may appear early in Oral Mucosa

Future lung cancer prevention trials, for example, could soon be designed so that surface tissues inside the cheek could be checked to detect tobacco-induced damage in the lungs, according to a study led by Mao last year published in the journal Cancer Prevention Research.

"We hypothesized that tobacco-induced molecular alterations in the oral epithelium are similar to those in the lungs," says Mao. "This might have broader implications for using the mouth as a diagnostic indicator for general health."

He added that for difficult extractions, for example, a badly broken tooth or a tooth with multiple or curved roots, it is not uncommon to fracture (break) the roots during extraction.

'Therefore breaking roots during extractions is not considered a 'mistake'. However, the most important thing is what the dentist does after the roots break,' said Dr Ang.

If the dentist is able to, she should try to take out the roots.

(Reuters Health) - Some of the antibiotics used to treat urinary tract infections during pregnancy may increase the risk of several birth defects if a woman uses them early in pregnancy, a new study in the Archives of Pediatrics & Adolescent Medicine shows.

The authors reviewed articles in the literature that addressed attaining primary stability of dental implants. They combined their findings with their own clinical experiences to produce the information provided in this article.

Results. The authors present practical information regarding bone density to delineate for clinicians instances in which modifications of the drilling protocol are needed. They also correlate measures to enhance primary stability with the alveolar bone density at the implantation site.

Clinical Implications. To enhance primary implant stability, modifications of the drilling protocol are necessary in different bone densities.

A team, led by Dr. Mark Little at Imperial College London, constructed a mathematical model to find the risks associated with low background levels of radiation.

Cost of bone grafting, dental filling to come down in India
The cost of medical treatment involving bone grafting or dental filling could come down in India with the Central Glass and Ceramic Research Institute (CGCRI) Sunday signing an agreement for low-cost application of the technology with private player IFGL Refractories Limited.
In the latest instance of institute-industry tie-up, the CGCRI - a constituent laboratory of India’s largest research and development conglomerate Council of Scientific and Industrial Research (CSIR) - signed an agreement with IFGL for the low-cost application.

The product was likely to be ready for commercial production within nine months, said IFGL director and chief executive Pradeep Bajoria, at the signing ceremony which coincided with the 67th CSIR foundation day. Full Report

How to Double the rate of orthodontic tooth movement ?

One method used to accelerate orthodontic tooth movement is the corticotomy-facilitated (CF) technique. The purposes of this study were to (1) identify the effect of the CF technique on orthodontic tooth movement compared with the standard technique, and (2) explore the histologic basis of the difference between the 2 techniques.
The first premolar on the CF side moved significantly more rapidly (P <0.05). Histologic findings showed more active and extensive bone remodeling on both the compressive and tension sides in the CF group.
Conclusions The CF technique doubled the rate of orthodontic tooth movement. Histologically, the more active and extensive bone remodeling in the CF group suggested that the acceleration of tooth movement associated with corticotomy is due to increased bone turnover and based on a regional acceleratory phenomenon (RAP).

A Michigan woman with severe periodontitis died this month because the state eliminated adult dental Medicaid benefits, according to the director of a chain of clinics that attempted to arrange her treatment.

When a physician detected her blood infection in early July, he recommended that all her teeth be extracted. The dental clinic scheduled this treatment at a local hospital, but the hospital postponed the procedure because she had mild pneumonia that prevented the use of general anesthesia.

By the time she recovered from the pneumonia, the state had eliminated dental services and the hospital would not put her under general anesthesia because the cost -- which it estimated at $4,500 to $6,000 -- would not be reimbursed by the state, Dr. Veryser said.

"Our hospital coordinator tried to come up with Medicaid reasons to force the hand of Medicaid," but the Medicaid office wouldn't budge, he said. More

The findings of the study reported in the journal Nutrition Research are expected to counter the beliefs of all those who think that red wine causes more harm.

The study suggests that white wines, such as chardonnay and pinot grigio, are more likely to cause damage to the protective layer of the teeth called tooth enamel than red wine like rioja and merlot.

Which Retainer is better ? Glass fibre or Multistrand

The objective of this study was to compare glass fibre reinforced (GFR) with multistranded bonded orthodontic retainers in terms of success rate and periodontal implications.

GFR retainers showed unacceptably high failure rates in comparison with multistranded retainers (51 versus 12 per cent). The most significant periodontal conditions were found in patients with GFR retainers with no significant differences between the GFR500 and the GFR1000 group for any parameter at any time point. Subjects without retainers showed significantly lower levels of gingival inflammation and plaque accumulation when compared with patients in any retainer group.

Multistranded retainers should remain the gold standard for orthodontic retention, although periodontal complications are common. The use of GFR retainers should be discouraged in daily practice.

Sugary medicines harm kids teeth

Nigel Carter, chief executive of the British Dental Health Foundation (BDHF) is alerting health professionals and the public on the decaying effects following a report by specialists at a Dublin hospital.
Doctors and healthcare professionals should prescribe sugar-free liquid medicines or medicines in tablet form if possible. If the medicine is only available in a sugar-containing liquid formulation, advice should be given to the child's parents and carers.

Dr Carter added weight to this argument, saying: ‘Patients and practitioners need to have a greater awareness of the risks of sugar containing medications particularly when these are taken regularly for chronic conditions and to look for sugar-free alternatives.' More HERE

Risk factors for the development of Enamel defects

Subjects and methods: Children between 4 and 5 years of age (n = 102) were classified into: Group 1) 52 low birth weight (<2500 g); Group 2) 50 normal birth weight (≥2500 g). Medical history, prenatal and neonatal variables were collected. Enamel defects were evaluated with the modified Developmental Defects of Enamel Index.

Results: The prevalence of hypoplasia and average number of affected teeth were significantly higher in group 1 than in group 2 (59.6%vs 16% and 1.6 vs 0.3 respectively). Low gestational age was linked to a higher prevalence of hypoplastic (P = 0.027) and combined defects (P = 0.001). Children with neonatal risk factors (low Apgar scores, parenteral nutrition, orotracheal intubation, mechanical ventilation and acidosis) developed defects more frequently (P < 0.05). Defects were symmetrically distributed in children who were not intubated; in those who required intubation they concentrated on the left maxillary teeth (P < 0.05). Smoking during pregnancy, young maternal age and multiple birth were significantly associated to developmental defects.

Conclusions: The prevalence of enamel defects in primary dentition is significantly influenced by birth weight, gestational age and several systemic factors. Orotracheal intubation probably plays an important role as a result of laryngoscope trauma on the maxilla.

Daily chewing gum exercise for stabilizing the vertical occlusion

We investigated the effects of a chewing gum exercise program on occlusal conditions and evaluated compliance of subjects. Thirty-five healthy adult volunteers (26 males and nine females) were asked to chew gum for 10–15 min before or after three meals daily for four weeks. Occlusal conditions were recorded as occlusal parameters, such as occlusal contact area, occlusal contact force, and pressure using dental prescale films. These parameters were evaluated by an Occluzer before the exercise period commenced, after four weeks of exercise, and then one month after the end of the exercise period. These parameters were statistically compared using one-way ANOVA. We found that: (i) after four weeks of exercise, anterior and posterior occlusal contact areas and forces were significantly (P < 0·05) increased and the increments were significantly (P < 0·05) higher in the anterior occlusal contact area and force than in the posterior occlusal contact area and force, (ii) the anteroposterior ratio of occlusal contact area and force increased, but not markedly, (iii) increased parameters had significantly (P < 0·05) decreased within one month after the end of the four-week exercise period, (iv) most participants did not complain for discomfort or stress during the exercise. The chewing gum exercise program could increase occlusal contact area and force and also move the anteroposterior occlusal balance forward. Patient compliance with the exercise is likely high enough to keep them exercising.

Dental students, teachers killed in car-bus mishap

SALEM: Two final-year BDS students and a dentist were killed on the spot and another dentist was seriously injured when the car in which they were travelling collided with a TNSTC bus here on Tuesday evening.

According to preliminary reports, Arathi Srikumar, 21, Sabari, 25, and Moshin (all from Kerala), and Fathima Suliya, 22, from Malaysia were returning from the Vinayaka Missions Dental College at Seeragapadi to their rooms when the accident occurred.

Arathi and Fathima were final year BDS students at the college while Sabari and Moshin were working as teaching staff in the same institution’s dental college hospital. At 4.30 pm , the four were on their way back from the hospital in a Maruti Swift car. Arathi and Fathima occupied the rear seat while Moshin took the left front seat. Sabari drove the vehicle. Nearing the Dream Land theme park on Salem- Coimbatore highway at Policekaran Thottam, a TNSTC bus coming from the opposite direction hit the car.

The car was mangled and the occupants were trapped inside.

Arathi, Sabari and Fathima died on the spot while Moshin, who was seriously injured, was admitted to the Vinayaka Missions Hospital. Moshin’s age could not be ascertained. The bodies were sent to Salem GH.

Salem SP N M Maylvahanam visited the hospital and made inquiries. Driver of the bus is absconding.

Dental assistant gets five years for fondling patient in USA

A dental assistant who sexually assaulted a woman during oral surgery will spend five years in prison, a judge ruled Wednesday in Frederick County Circuit Court.

On April 22, the 23-year-old patient was under anesthesia to have wisdom teeth removed. She awoke to find Danilo Carabio Banados kissing her mouth and fondling her under her clothes, authorities said.

"You violated (her) trust," Judge G. Edward Dwyer Jr. said, explaining his decision to go beyond the state sentencing guidelines of probation to one year.

"Mr. Banados, you came in here at most with a minor record," Dwyer said. "Now you have two felonies that will follow you for life." Full report with Pic

Survival and failure rates of orthodontic microimplants: a systematic review

Based on the available evidence in the literature, palatal implants and miniplates showed comparable survival rates of >=90% over a period of at least 12 weeks, and yielded superior survival than miniscrews. Palatal implants and miniplates for temporary anchorage provide reliable absolute orthodontic anchorage. If the intended orthodontic treatment would require multiple miniscrew placement to provide adequate anchorage, the reliability of such systems is questionable. For patients who are undergoing extensive orthodontic treatment, force vectors may need to be varied or the roots of the teeth to be moved may need to slide past the anchors.

How hard is the skin of your TEETH ?

It's been a mystery: how can our teeth withstand such an enormous amount of pressure, over many years, when tooth enamel is only about as strong as glass? A new study by Prof. Herzl Chai of Tel Aviv University's School of Mechanical Engineering and his colleagues at the National Institute of Standards and Technology and George Washington University gives the answer.
"Teeth are made from an extremely sophisticated composite material which reacts in an extraordinary way under pressure," says Prof. Chai. "Teeth exhibit graded mechanical properties and a cathedral-like geometry, and over time they develop a network of micro-cracks which help diffuse stress. Periodontal Therapy Improves Gastric Helicobacter pylori Eradication

The oral cavity has been proposed as a reservoir for H. pylori that could be responsible for the refractoriness of gastric infection to triple therapy (antibiotics, antimicrobials, and proton pump inhibitors). The aim of this study was to evaluate the efficiency of combined periodontal and triple therapy vs. triple therapy alone, in gastric H. pylori eradication in persons with H. pylori in the subgingival biofilm. Individuals positive for H. pylori in their gastric and oral samples, as determined by nested PCR, were treated either with periodontal and triple therapy or with triple therapy alone. Our results indicate that 77.3% of those treated with the combined therapy exhibited successful eradication of gastric H. pylori, compared with 47.6% who underwent only triple therapy. Analysis of these data suggests that periodontal treatment in combination with systemic therapy could be a promising approach to increasing the therapy’s efficacy and decreasing the risk of infection recurrence.

Gujarat government to introduce laser education in dentistry
Society For Oral Laser Application (Sola) affiliated to International Society for Oral Laser Applications in Vienna is working on a project on laser education in Government Dental College in state capital, its secretary Mohan Vakade said today.
The move comes after keen interest shown by Gujarat health minister Jjaynarayan Vyas in laser education who emphasised on the need of it after releasing a book on clinical guide for Oral Laser Applications authored by Vakade, Anil Shah and others at the 3rd national conference on oral laser applications held in Ahmedabad earlier this month. University-Level Dental Sleep Medicine Program Opens at Tufts
Tufts University, Boston, has developed the first university-level Dental Sleep Medicine Program in the United States. Starting in fall 2009, the Craniofacial Pain, Headache and Sleep Center, part of the Department of General Dentistry at Tufts Dental School, will offer the new Dental Sleep Medicine (DSM) course to postgraduate dental students.

The course will provide classroom and clinical instruction to postgraduate students. Postgraduate students and residents can participate in patient assessment, interpretation of polysomnograms (an overnight lab test used in to diagnose sleep disorders), and the use of ambulatory sleep study monitors when titrating oral appliances. The students can also be trained in selecting, fitting, and adjusting the oral appliances used to treat obstructive sleep apnea.
The American Academy of Dental Sleep Medicine (AADSM) applauded Tufts University School of Dental Medicine for its development of the flagship Dental Sleep Medicine Program.

Periodontitis as a risk factor for cerebrovascular accident: a case–control study in the Indian population

This case–control study consisted of 200 subjects (100 cases and 100 controls) who were 33–68 years of age. Cases were examined within 5 d after their first acute ischemic attack. Clinical parameters recorded included plaque index, gingival index, probing pocket depth and clinical attachment loss. The medical questionnaire collected information on family history of stroke, dietary history, history of smoking and history of alcohol consumption. Cases were additionally scrutinized for diabetes, hypertension and total serum cholesterol level. The education level of patients was also considered.

Results: The mean plaque index, gingival index, probing pocket depth and clinical attachment loss values of subjects with cerebrovascular accident were significantly higher when compared with those of the control group (p < 0.05). In fully adjusted logistic regression analysis, a probing pocket depth of > 4.5 mm was found to be the most significant factor for stroke (odds ratio = 8.5; confidence interval = 1.1–68.2) followed by hypertension (odds ratio = 7.6; confidence interval = 3.3–17.1) and smoking (odds ratio = 3.1; confidence interval = 1.3–7.4).
Conclusion: The data from this study support the proposed link between periodontitis and cerebrovascular accident in the Indian population. However, further studies are necessary to verify and quantify the role of oral infections and genetic factors in the process of atherosclerosis.

Dental plaque increases neutrophil activity
The researchers, led by Michael Kowolik, B.D.S., Ph.D., professor of periodontics and associate dean for graduate education at the IU School of Dentistry on the campus of Indiana University-Purdue University Indianapolis, studied 128 black and white men and women and found that dental plaque accumulation did not result in a change in total white blood count, a known risk factor for adverse cardiac events. However, in black males the researchers noted a significant increase in the activity of neutrophils, the most common type of white blood cell and an essential part of the immune system.

Unlike most other studies that attempt to understand the link between oral inflammatory disease and heart disease risk, these study participants did not have periodontal disease. They were healthy individuals who by the study design were asked to neglect oral hygiene. More HERE

Lollipop contributed to poor dental health, damages filed in USA

Earlier this month a man from Texas filed a lawsuit against a company and claimed their Actiq fentanyl lollipop negatively affected his dental health by rotting his teeth.

Greg Lee Minckley alleges in the suit that the sugar in the lollipop "destroyed" his teeth and forced him to get $37,000 in dental care to remedy the problem. The lollipops were designed as a way to administer pain medication through a lozenge at the end of the stick for cancer patients.CLICK HERE

Which Light is better for Bonding ortho brackets ?

The purpose of this study was to assess the influence of two light units, a quartz–tungsten–halogen (QTH) and a light-emitting diode (LED), on the hardness and degree of conversion of an orthodontic composite resin. Sixty specimen disks were prepared from Transbond XT composite resin (3M Unitek) and light cured for 10, 20, and 30 seconds with a QTH (Curing Light XL 3000, 3M Unitek) or a LED (Ortholux, 3M Unitek) light-curing unit for 5, 10, and 15 seconds.

The highest Knoop hardness number (KHN) was obtained with the QTH at 30 (25.19 KHN) and 20 (24.01) seconds, which did not differ statistically, and in the LED 15 second (21.86) group. The QTH 10 second group (20.53) did not differ statistically from the QTH 20 second or the LED 5 (19.96) and 15, or 10 second (18.95) groups. According to FTIR, there was no statistical difference for the degree of conversion among the groups, QTH 10 (43.42 per cent), QTH 20 (46.12 per cent), QTH 30 (45.30 per cent), LED 10 (47.02 per cent), or LED 15 (47.24 per cent) seconds.

Light curing with the LED resulted in a reduction of 50 per cent in the time recommended for use of the QTH light with the composite resin, Transbond XT.

Effect of Thermoplastic retainer (Essix) on occlusal contacts

Essix Retainers are great for retention. No significant change was observed in the number of posterior contacts during T1(9 months of retention; T1), whereas a significant increase was found at T2 (2.5 years) (P < 0.01) for the second premolars (P < 0.01) and second molars (P < 0.05). Both ‘ideal’ and ‘non-ideal’ contacts increased significantly but only at T2 (P < 0.05). The number of ideally located contacts on the posterior teeth at all three periods were lower than normal values (P < 0.01); while non-ideal contacts at T1 (P < 0.05) and T2 (P < 0.01) were found more often when compared with the values of normal subjects. Only the increased number of premolar contacts at T2 was more than normal values (P < 0.01). There was no expected increase in occlusal contacts at T2.

Sexual posturing with patient in Dental Chair, has been an historical issue/debate. Roger Marcus Frazer Klein, who practices in Knutsford, Cheshire, was disciplined by the General Dental Council (GDC) on August 21, after he was charged with the assault.

The GDC slapped 13 conditions on him for a period of 18 months following August's hearing.
As well as being unable to treat women without a chaperone – unless it is a life-threatening emergency – 41-year-old Mr Klein must keep a log of every consultation he has with a female patient, which must be signed by a chaperone.

He cannot be at his Knutsford dental surgery, Regent Cosmetic Dental Practice, outside of practice hours (except in an emergency), or with fewer than two colleagues there.
Mr Klein, from Ladyacre Close in Lymm, also cannot be alone with any female member of staff at the practice.MORE

Two Abu Dhabi Dental Clinics Accused of Malpractices

The National Health Insurance Company or Daman has taken action against two dental clinics for overcharging and indulging in  malpractices.

Daman Chairman Khaled Abdulla Al Qubaisi said that immediate action was taken against clinics once the cases were discovered. He added that under no circumstances will any form of abuse or fraud be tolerated by Daman.

Health Ministry recommends to scrap Medical Council and Dental Council of India
The Health Ministry had proposed to scrap the Medical Council of India (MCI) and Dental
Council of India (DCI), citing alleged corruption in these regulatory bodies.

The proposal was sent to the Prime Minister's Office for listing it before the Cabinet so that a necessary amendment could be moved in the next session of Parliament.

The PMO, however, rejected the proposal, saying there was no surety that the new body replacing MCI and DCI would not develop shortcomings as of the present regulators, an official said. MORE

New Device Could Shorten, Improve Orthodontic Treatment

A team of orthodontists at The University of Texas Health Science Center at San Antonio hopes to speed up and improve orthodontic treatment using a new device called the AcceleDent.

Dubravko Pavlin, D.M.D., M.S.D., Ph.D., an associate professor in the Department of Orthodontics at the Health Science Center, worked with a team of investigators from OrthoAccel Technologies, Inc. of Houston to help develop the device.

The study involves 34 patients ages 12 to 40 who need braces. Patients receive standard orthodontic treatment including any necessary extractions, and are fitted for braces at the Health Science Center Orthodontic Clinic. Once their braces are on, patients are provided with an AcceleDent device to use at home. The device, which stands about 4 inches high and features a wafer-thin mouthpiece attached to a small, sleek battery-operated handle, almost resembles a gizmo from a science fiction movie.

Patients are instructed to place the device in their mouth and bite down for 20 minutes a day during orthodontic treatment. While in the mouth, the device delivers a low-frequency pulsating vibration to the teeth. The researchers believe the vibratory forces, in combination with standard orthodontic treatment, will enhance the metabolism in bone tissue surrounding the teeth, thus allowing the teeth to move and straighten faster and more effectively. This study evaluated whether mobile phone induces physiologic changes in the adjacent parotid gland, located on the dominant side, in terms of secretion rates and protein levels in the secreted saliva.
Materials and method: Stimulated parotid saliva was collected simultaneously from both glands in 50 healthy volunteers whose mobile use was on a dominant side of the head.

Results: A significantly higher saliva secretion rate was noticed in the dominant mobile side compared with that in the non-dominant side. Lower total protein concentration was obtained in the dominant compared with the non-dominant mobile side among the right dominant mobile users.
Conclusions: Parotid glands adjacent to handheld mobile in use respond by elevated salivary rates and decreased protein secretion reflecting the continuous insult to the glands.

The girl who went missing is Anamika Yadav of Allahabad, a first year student of BDS from Saraswati Dental College in Chinhat. Talking to TOI, Anamika's father, R A D Yadav said, "She had come to Hazratganj on Thursday evening along with her other college mates to purchase some books. What her friends told was that about 8.15pm she went to purchase some medicines from a nearby medical store but did not return."

Prevent Periodontitis To Reduce The Risk Of Head And Neck Cancer
Chronic periodontitis, a form of gum disease, is an independent risk factor for head and neck squamous cell carcinoma. This suggests the need for increased efforts to prevent and treat periodontitis as a possible means to reduce the risk of this form of cancer.
Results of this study are published in Cancer Epidemiology, Biomarkers & Prevention, a journal of the American Association for Cancer Research.
Findings showed that chronic periodontitis might represent a clinical high-risk profile for head and neck squamous cell carcinoma. The strength of the association was greatest in the oral cavity, followed by the oropharynx and larynx, according to Dr Mine Tezal.
The study of poor oral health including the possible carcinogenic role of microorganisms is part of a rapidly growing interest in how a community of microbes that live in the various environments of the human body can affect health. Full News

Manipal leads in Innovation on indigenous Implant

September 7: Manipal University team presented a business plan on indigenous dental implants to bag the third prize at the prestigious India Innovation Pioneers Challenge 2009 held at Intel Corporation in Bangalore on July 14. The prizes were distributed by Mr Prithviraj Chavan, Minister of state for Science and Technology at a function in New Delhi on August 28.

There were over 190 entries for the competition and 18 teams were short-listed for a final round.

Samit Ashok Shah failed to disclose he was under investigation by the police when he applied to the General Dental Council (GDC) for registration on the Dentists Register and to the London Deanery for the position of Specialist Registrar in Dental Public Health.

Mr Shah also failed to disclose the criminal proceedings which led to his conviction on his applications for employment to the Hillingdon Primary Care Trust (PCT) and the St. Mary's NHS Trust.

The Professional Conduct Committee heard on 4 August at Southwark Crown Court in London, that the money was returned to his employer in full as soon as Mr Shah's actions came to light. MORE HERE

Retention of Metal-Ceramic Crowns-Which Cement is better

Powder-liquid versions of new cements were significantly more retentive than were paste-paste versions of the same cements. The mean value of crown removal stress for the new self-adhesive modified-resin cements varied appreciably among the four cements tested. All cements retained castings as well as or better than did zinc phosphate cement.

Clinical Implications. Powder-liquid versions of cements, although less convenient to mix, may be a better clinical choice when crown retention is an issue. All cements tested will retain castings adequately on ideal preparations because the corresponding removal stresses are comparable with or higher than those associated with zinc phosphate. Powder-liquid resin-modified glass ionomer cement, selected self-adhesive modified-resin cements and conventional resin cements provide additional retention when desired.

JAIPUR: A day after a ragging incident was reported at the Jaipur Dental College reported on Wednesday, authorities claimed it was a case of dispute among the students over a common love interest.

However, the victims have alleged they were ragged by their batchmates."There has been no official complaint by any of the students, yet after the issue was brought to our notice, the management is investigating into the matter. At present, college romance' seems to be the possible cause behind the clash," Dr Durgesh N. Bailoor, principal, Jaipur Dental College, said.

SAN DIEGO - A Paradise Hills woman pleaded guilty Tuesday to four felony charges for stealing about $72,000 from the bank accounts of patients of several area dental offices where she worked as office manager.

Emily Marie Forbes, 48, will be sentenced to five years and four months in prison on Oct. 1, with a recommendation that her custody be served in a Department of Corrections drug rehabilitation program.

Woman awarded £1.4million for BAD dentistry

A jury in South Carolina has awarded damages in the amount of £1.4 million ($2million) to a woman who had 13 of her teeth mistakenly extracted by a dentist.

Back in 2006, Elizabeth Smith, aged 28, attended the Sexton Dental Clinic for dental treatment involving the extraction of three teeth. For some reason, the dentist proceeded to remove all of Ms Smith's upper teeth, resulting in a total extraction of 16 teeth.

Ms Smith's lawyer, Robert Ranson, said his client is now looking forward to having remedial dental work performed, which, it is thought, will cost in the region of £50,000 ($80,000).

Mr Ranson had claimed that the clinic's employees tried to cover up the mistake by altering the paperwork.

The Sexton Dental Clinic denies any wrongdoing and is considering appealing the judgment.

Implant-supported fixed cantilever prosthesis in partially edentulous jaws: a cohort prospective study

The aim of the present study was to assess the clinical outcome of patients treated with implant-supported fixed partial dentures (FPD) with cantilever after a mean follow-up time of 8 years.

The study included 45 consecutive partially edentulous patients treated between January 1994 and August 2006 with 59 partial cantilever fixed prostheses supported by 116 ITI® implants. The primary outcome variable considered was the presence of complications at the subject and bridge level; the secondary outcome variable was marginal bone loss (MBL). The frequency of complications was analyzed according to cantilever location and opposite dentition and tested by Fisher's exact test. A multilevel regression model was constructed to analyze the factors influencing MBL with three levels: subject as the highest, and then implant and site. During the follow-up period, 11 implants showed a bone loss exceeding the limit for success, out of which two implants showed an infection of the peri-implant tissue.
Results: After an average observation of 8.2 years of cantilever prostheses loading, the implant success and survival rates were 90.5% and 100%, respectively. Besides, the prosthetic success and survival rate were 57.7% and 100%, respectively.
None of the predictors included in the multilevel model presented a significant impact on the bone loss between baseline and the follow-up examination.
Conclusions: The authors concluded that the prognosis of implant-supported FPDs and marginal bone loss at implants were not influenced by the position or the length of the cantilever, the location of the bridge and type of opposite dentition. Implant-supported fixed cantilever prosthesis can be considered a suitable treatment choice.

Service Tax on Orthodontists

This is to inform all the life members of the IOS that the Government Of India has proposed to levy a service/sales tax of 10.3% on orthodontic treatment from 01st Sept'09. The Head Office has already dispatched a letter to Government of India protesting against this tax being implemented on this specialty and requested them to reconsider this taxation.

The head office is also sending a representation to the Director General ,Tax Department to plead and represent the case in front of them and convince them that it is not a cosmetic treatment.

The members are requested to kindly await further guidelines regarding this issue and refrain from paying this tax till this matter is resolved as it would weaken the representation of the Association in front of the Government of India. A Chinese dentist has built a tower of rotten teeth in his waiting room to remind patients about good oral health.Yu Qian, from Beijing, displays the rotten ones he's pulled from his patients' mouths in his waiting room in a resin-cast tower.He has used the pulled 28,000 teeth to build his tower which is more than eight feet tall.
