Are you Doing the RCT Irrigation in the Right Way ?

Did you know that the irrigation that you do for your root canals is positive pressure irrigation, it is dangerous and NEVER Reaches in the apical area, read on...

For years the dictum was "shaping the root canals with instruments and cleaning with Irrigants".But there was no scientific proof to demonstrate the effectiveness of irrigants as cleaning & disinfecting agents, especially the apical third area of root canals.

This is especially critical with the present day trend to finish instrumentation with #20 or #25. Now there is scientific evidence that there is an apical air pocket that prevents irrigant from reaching this area. Sodium hypochorite is a very effective irrigant and solvent of necrotic tissue but it can not act unless it has surface contact and there is good volume used to flush out the debris.

Patency is one of the most important factors in success of endodontic treatment. Did you know that most of your instruments can not remove pulp tissue completely because their tip size is mere 20 or 25. Only instrument that scoops out the pulp completely is LSX,
In inflamed cases you will be amazed to see the result, bleeding immediately stops and patients does not complain of pain during further instrumentation. This excellent article discusses the issue of patency.

Number of teeth - a predictor of mortality in 70-year-old subjects
Community Dentistry and Oral Epidemiology (OnlineEarly Articles) Oct 2007

Objectives: To investigate whether the number of teeth at age 70 is an independent predictor of mortality.

Methods: Within the gerontological population studies in Göteborg, Sweden, four birth cohorts born in 1901/1902, 1906/1907, 1911/1912 and 1922 were examined cross-sectionally at 70 years of age. The total number of participants in the odontological cohorts was 1803. Mortality data were collected from the national Swedish health registers. Cox regression models were used to measure the association between mortality and the number of teeth with adjustment for covariates such as health factors, socio-economic and lifestyle factors.

Results: The prevalence of edentulism showed a marked change from 51% in the first cohort to 16% in the last cohort. The 7-year mortality rate was 14% in women and 28% in men, and the highest in edentulous men in the last two cohorts (42% and 47% respectively). The 7-year mortality including all four cohorts showed a hazard ratio of 0.96 (95% CI 0.94–0.98; P < 0.001) for the number of teeth with adjustment for cohort. The corresponding 18-year mortality including the three first cohorts showed a hazard ratio of 0.98 for women and 0.97 for men. The number of teeth was an independent statistically significant predictor of 7-year mortality in both genders and of 18-year mortality in men.

Conclusions: The result showed that each remaining tooth at age 70 decreased the 7-year mortality risk by 4%. The difference between edentulous subjects and dentate subjects with ≥20 teeth regarding 7-year mortality was significantly higher in the last compared to the first cohort. The number of teeth was a significant predictor of mortality independent of health factors, socio-economic status and lifestyle.

ACP - will it replace Fluoride, Tooth Migraine

Fluoride has ruled for a long time, is it time for change ? Can your tooth get migraine ? Read on to find out.

Amorphous calcium phosphate (ACP) is more than a cosmetic benefit for patients—it's the first nonfluoride therapy to remineralize enamel and dentin.

From healing the early signs of caries to desensitizing teeth to adhering orthodontics, ACP is one of the hottest materials in dentistry.

The compound's potential was discovered in 1991 by Ming S. Tung, Ph.D., chemist and researcher at the ADA Foundation Paffenbarger Research Center in Gaithersburg. PRC scientists believe that ACP is on the cusp of becoming commonplace in the dentist's office.

"This is the next big thing," said Clifton M. Carey, Ph.D., director, Independent Research and Grant Administration at PRC. "This is the first big thing that could replace fluoride."

Said Dr. Daniel Meyer, senior vice president, ADA Science/Professional Affairs: "It has the potential of being a major ingredient in products designed for remineralizing hard tissues, including bone, enamel and dentin. It depends not only on the material, but the way products are designed along with the mechanisms to deliver the material to the appropriate areas.

Because of its unique properties ACP is being incorporated in many gels and composites, tooth whitening agents etc. It is also part of GC 's popular Tooth Mousse.

Is there a primary neuromuscular pain originating from facial area ? Some patients present with pain over maxillary sinuses but no supporting physical or radiographic findings supporting diagnosis of sinusitis. In absence of pathology these signs and symptoms may indicate a primary neuromuscular pain in lower or mid face area. To read the full editorial Click Here.

I came across another very interesting website which is of help when you have confusing radiographic lesions, enter the details on the website and it will give you possibilties, preety good, have a look. http://www.orad.org

Make your Endo always Enjoyable

For all of us Endo is an important part of our clinical work.
Facing problems in your endo cases ? Time for introspection, do you know the latest trend/ paradigm is now larger apical preparations, when you have biologically optimal preparations you have a happy patient.

Do you want to be guilty (may be get sued) of not following the best practice accepted worldwide. It is a well accepted fact that larger MAD (master apical diameter) leads to cleaner canals.

It has been found that majority of dentists who are using tapered instruments fail to clean the apical area and hence the failure and post op problems in number of their cases. There is an easy solution for you, rather very quick to adopt. To read a wonderful article published in USA CLICK HERE. (change your endo life). Do it and experience the difference and joy. We are waiting for your feedback.

Six per cent of people in a large survey in England said they had to treat their own dental problems themselves, because they could not get professional treatment - this included pulling out their own teeth. How does India compare with UK ?
The Dentistry Watch survey, which is the largest of its kind ever to be carried out in England, surveyed 5,212 patients and 750 dentists. It was carried out by the Patient and Public Involvement Forums survey and was aimed at collating patients' and dentists' views on dental services in England today and assessing the true state of the National Health Service dental system. Times are great for dentists in America, The average American adult patient now spends roughly $600 annually on dental care, of course half of this is paid by the insurance companies. What would you say for India ? Full article here

Time to teach public "must get dental checkup every 6 months"

Mouth is the portal of body. Without healthy mouth it is impossible to have good general health and self esteem. This fact is to be made known to public. It is high time that a systematic campaign is taken up to drive this point home. People do a servicing for their car and two wheeler, they have to be educated that 6 monthly dental checkup is a must. Here are two more news items, one of them is from Harvard.

For many, good oral care—not antibiotics—best bet in preventing dental-related heart problems, reports the Harvard Heart Letter

Since the 1950s, the American Heart Association (AHA) has urged a sizable group of people to take antibiotics before having dental work or other procedures that might flood the bloodstream with bacteria. The antibiotics were supposed to prevent infective endocarditis, a potentially serious infection of the heart’s lining. After a look at the latest evidence, the AHA now emphasizes routine oral care and recommends pre-procedure antibiotics for only some people, reports the October 2007 issue of the Harvard Heart Letter. If good oral health is maintained heart is always healthy and it is better than taking antibiotics. Full report here

Study Finds Possible Link Between Tooth Loss, Dementia, Tooth loss= Memory loss

Tooth loss may predict the development of dementia late in life, according to a study by University of Kentucky researchers.

The researcher, led by Pamela Sparks Stein in the UK College of Medicine's Department of Anatomy and Neurobiology, used data collected from 144 participants in the Nun Study, a study of aging and Alzheimer's disease among Catholic sisters of the School Sisters of Notre Dame.

"Of the participants who did not have dementia at the first examination (of annual exams over a 12-year period), those with few teeth zero to nine had an increased risk of developing dementia during the study, compared with those who had 10 or more teeth," the researchers reported in a paper published in the October issue of the Journal of the American Dental Association (JADA). The researchers relied on dental records and annual cognitive exams of the Nun Study participants in the order's Milwaukee province. The participants' ages ranged from 75 to 98 years old. More here
